Community Nursing in the Northeast

Sengkang General Hospital (SKH) has recently launched its Community Nursing services in the Northeast region of Singapore, focusing on two
Communities of Care: Sengkang and Punggol.

Services provided

The SKH community nurses provide the consultation and services through either the Community Nurse Posts (refer to locations below). Home Visits for seniors who are homebound. In addition, video-consultation was recently introduced to ensure continuity of care for seniors, especially during the pandemic.


Eligibility Criteria:

  • Seniors aged 60 years old and above;
  • Have multiple chronic diseases (e.g. high blood pressure, high blood cholesterol, diabetes, chronic heart failure, chronic lung diseases
  • or stroke) and need help to manage their conditions; and
  • Singapore citizens or permanent residents

Pre-Frail and Frail seniors with chronic conditions are eligible to consult the community nurses and the following services will be performed

depending on seniors' needs:

  • Health and geriatric assessment;
  • Health coaching;
  • Chronic diseases monitoring and education;
  • Medication education; and
  • Care referral and coordination.

Apart from the above services, the programme also provides regular health talks at the selected SACs, CCs and RCs.

Referral to CNP

Medical professionals and social service partners can refer their patients or clients who needs community nursing services by filling up the following document Community Nursing Programme Referral and send it to .

The CNP team will review the referral form and inform the senior about their enrolment within 3 working days, either by email/ phone call.
