Advance Care Planning

​Advance Care Planning (ACP) is a voluntary discussion to plan for your future healthcare decisions. It takes into consideration your wishes, goals, personal values and beliefs. By making your wishes known ahead, your loved ones will understand which form of care you would want (or not want) if you become very sick and are not able to make decisions in the future. The ACP discussion would serve as a guide for your loved ones and healthcare providers to help make healthcare decisions for you. You will also be asked to appoint a nominated healthcare spokesperson (NHS) to help you to make important decisions in the event that you are no longer able to communicate.

Who is ACP for?

Everyone – Regardless of age or health condition. Nobody can predict the future.

Start thinking about this when you are healthy! Let’s not wait till it’s too late to make plans. With ACP, you can have a say in your own care plans.
