Accessing Your Discharge Documents

You can access your discharge documents on the Health Hub app without waiting for them to be printed. If you require follow up at our specialist clinics, your appointment date and time will be sent to you via SMS. 

Access Your Discharge Information on Health Hub in 3 Easy Steps

Step 1: Download the Health Hub app from App Store or Play Store. Login to the Health Hub app with your SingPass.	Step 2:  Select My Health Booklet.	Step 3:  Select Discharge Information.
