SingHealth Regional Primary Care Network (SHR PCN)
The SingHealth Regional Primary Care Network (SHR PCN) supports Singapore’s Healthier SG vision by strengthening primary care and enabling patients to receive care closer to home.
Established under the Ministry of Health’s (MOH) Primary Care Network (PCN) scheme, SHR PCN brings together a network of private General Practitioner (GP) clinics to deliver coordinated, team-based care in the community. This approach supports the national shift towards preventive care, early intervention and improved management of chronic conditions outside the hospital setting.
How the PCN Model Supports GPs and Patients
Through the PCN scheme, participating GP clinics receive funding and administrative support to deliver multidisciplinary care in the community. Patients are supported by a care team comprising doctors, nurses and primary care coordinators, enabling more systematic monitoring, follow-up and care planning. The network also facilitates collaboration and sharing of best practices among participating GPs to support continuous improvement in care delivery.
Sengkang General Hospital (SKH) supports SHR PCN, partnering a network of private GP clinics and supporting them with a dedicated hospital-based team of nurses, primary care coordinators and administrative staff to strengthen care delivery in the community.
Community-Based Services for Patients with Chronic Conditions
To support comprehensive chronic disease management within primary care, SHR PCN works with participating GP clinics to provide and coordinate the following community-based services.
These services support early detection, regular monitoring and ongoing management of chronic conditions, contributing to better health outcomes for patients.
Working with Our GP Partners
SHR PCN works closely with participating GP clinics to support care delivery, coordination and capability development. Through ongoing collaborations and organising of CMEs to support ongoing professional development and updates in clinical practice, the network supports GPs in delivering accessible, coordinated and high-quality care for patients in the community.
