Which Patient Classification am I in?

You are a Subsidised Patient* if you meet ALL of these requirements:

  • Singapore Citizen or Permanent Resident
  • Referred by either of the following:
    • Polyclinic
    • Public hospital as a subsidised patient
    • CHAS GP Clinics as a CHAS cardholder (only applicable if you are a Singapore Citizen)
  • No preference specified for a named specialist

*Please note that as a subsidised patient, you will receive team-based care led by an assigned specialist and you may not be attended to by the same doctor for every visit. Rest assured that patients will receive the same quality of care regardless of their subsidy status.

 

You are a Private Patient if you meet any of the requirements:

  • Foreign resident or non-resident of Singapore
  • Referred by either of the following:
    • Public hospital as a private patient
    • Private practitioner or hospital
  • Have requested a specific specialist by name

What do I need to bring for my appointment?

  • Singapore NRIC, Birth Certificate / Health Booklet (if you are under 15) or Passport and Employment Pass / Work Permit / Dependent pass
  • Medical Benefit Card (e.g. Civil Service Card, Blood Donor Card, etc.)
  • Any other Medical Benefits Eligibility Card / documents (e.g. Letter of Guarantee)

Please bring along these additional items if this is your FIRST visit.

  • Referral letter from your polyclinic / other healthcare institutions / private doctor
  • Medications (if any)
  • X-ray and investigation records (within the last 6 months) (if any)

If you have any investigative tests to be done on the same day as your appointment, please bring along the original laboratory investigation forms provided during your previous visit to the hospital. To prepare for your investigative tests, please follow the specific instructions that were shared with you.
