The Community Health Post (CHP), supported by SKH’s Healthier SG (HSG) Team brings integrated health and wellness services directly into the community through our team of friendly Community Nurses and Well-being Coordinators. CHP focuses on delivering preventive care, health education, personalised coaching, and chronic disease management, while also providing care coordination and referrals to healthcare and community resources.
