SKH’s pharmacists are also committed to providing patients with the relevant information on their prescribed medications, ensuring that they take their medications regularly and work with the doctors to manage their medical problems.

The department also provides medication management services to patients who have difficulties with multiple prescriptions as well as to help resolve any drug-related problems.

Upon discharge from the hospital, pharmacists will dispense and educate patients/ caregivers on the proper use of their medications and things to look out for e.g., potential side effects and drug-drug/ drug-food interactions. The SKH pharmacy provides the following services:

Outpatient Pharmacy

The Outpatient Pharmacy serves patients at the Specialist Outpatient Clinics located at the Medical Centre. They review patients' prescription immediately after each consultation. The Outpatient Pharmacy also fills prescriptions from polyclinics, general practitioners, private hospitals and other restructured hospitals. It also provides counselling and consultation on minor ailments and the sale of pharmacy-only medication.

Inpatient Discharge Pharmacy

Inpatient Discharge Pharmacy serves hospitalised patients. The pharmacists review patients' list of medications ordered and provide support in optimising the outcome of therapies provided. They also dispense medications to patients upon discharge from the hospital.

Collecting Your Medications (for inpatients)

Upon discharge, your medication will be prepared at Inpatient Discharge Pharmacy and supplied to you by the pharmacist.

Location: Level 1, General Hospital (Next to Mr Bean.)

Emergency Pharmacy serves patients from the Emergency Department. The Emergency Pharmacy is open 24 hours a day and serves all patients discharged from the A&E department. Location: Level 1, General Hospital (behind Medical Centre)

Anticoagulation clinic

The Anticoagulation clinic provides services to outpatients by reviewing their anticoagulation orders so as to optimise therapeutic outcomes, as well as ensure patient's safety while on treatment. This service is strictly by doctor's referral only.

Medication for inpatients can be collected at Inpatient Discharge Pharmacy which is located on Level 1 of the General Hospital (next to Mr Bean?). Upon discharge from the hospital, your medication will be prepared at Inpatient Discharge Pharmacy and supplied to you by the pharmacist.
