SKH@Home provides an alternative approach to inpatient care that offers suitable patients the option to receive hospital-level treatment in the comfort of their homes. This helps to alleviate waiting times and reduces hospital bed occupancy promoting more efficient healthcare delivery. Managed by the department of Transitional Care and Community Medicine and supported by Internal Medicine specialists, this initiative helps ensure that patients receive high-quality care tailored to their individuals needs while remaining in their preferred environment.

A dedicated team of doctors, nurses, physiotherapists, occupational therapists, speech therapists and pharmacists will conduct both home rounds (visits) and virtual rounds via telemonitoring and teleconsultation for the patient.
After the patient is admitted to their home, a programme coordinator meets the patient at the site of the recruitment to brief them and the Next-of-Kin on what to expect during their enrolment. The programme coordinator will also pass the patient a set of remote monitoring devices for daily use. The care team comprising of doctors and nurses will also provide patient with daily reviews based on the patient’s clinical stability and the potential for deterioration. Close monitoring of the patient’s medical and psychosocial wellbeing will also be done. Additionally, private medical service providers are also working to deliver clinical and care services for patients at home.
The care team is accessible 24/7. During office hours, messages and calls will be answered by the SKH@Home staff or the doctor on-call. After office hours, phone calls and messages from patients will be answered by the SKH@Home on-call doctor who would assess the situation and follow up with an appropriate action.
From April 2024, SKH@Home has become a mainstream model of care. SKH@Home will be priced similar or lower than a normal hospital ward stay. Patients will be supported by subsidies, insurance and saving schemes such as the Integrated Shield Plan, MediShield Life and MediSave, no different from a physical inpatient stay.
