Sengkang General Hospital (SKH) has recently launched its Community Nursing services in the Northeast region of Singapore, focusing on two Communities of Care: Sengkang and Punggol.
The SKH community nurses provide the consultation and services through either the
Community Nurse Posts (refer to locations below). Home Visits for seniors who are homebound. In addition, video-consultation was recently introduced to ensure continuity of care for seniors, especially during the pandemic.
Pre-Frail and Frail seniors with chronic conditions are eligible to consult the community nurses and the following services will be performed
depending on seniors' needs:
