Ageing Successfully in Place - Independent, Rewarding Lives (ASPIRE)


     

What is frailty?

Frailty is a condition that results from the gradual loss of bodily reserves with age. Frail older persons may experience some or all of the following: tiredness, weakness, di­fficulty walking, loss of weight. Frailty increases the older person's risk of negative health-related outcomes such as falls, disability and hospitalizations. Not all older persons become frail. With proper care, an older person's frailty can be reversed or prevented from further deterioration.

How can ASPIRE help you?

If you are recommended for ASPIRE, you would have been identified to be frail. Following enrolment into ASPIRE, you will receive comprehensive assessment and management by a multi-disciplinary team of healthcare professionals. The team will put together an individualized care plan for the management of your frailty and related health conditions.

The multi-disciplinary team may include the following healthcare professionals, depending your specific needs:

  • Doctors
  • Nurses
  • Case managers
  • Physiotherapists
  • Occupational Therapists
  • Dietitians
  • Speech Therapists

The programme may last up to one year, and your medical team will inform you when you may be ready to be discharged from the programme. Once enrolled, you are encouraged to complete the programme, as frailty takes time to improve.

Frailty can be managed by addressing underlying medical conditions, exercise and with a proper diet.

How much will I need to pay?

As a new service, sessions are currently heavily subsidised and a relatively small fee is payable by patients. You will be advised of these fees at the time of enrollment. Medications and medical investigations prescribed by ASPIRE doctors are not included in these fees and these costs will need to be paid for separately.
