During the inpatient stay, there might be some documents which you, as the patient or caregiver, will like to submit for claims purposes. The documents that are applicable may include the following:

1. MediSave Authorisation Claims Form (MCAF-S) – to claim MediShield Life/MediSave and/or Integrated Shield Plan
2. Medical Benefit Cards
3. Letter of Guarantee
4. SingHealth Staff Benefit Card
5. Other documents which our Business Office staff had requested to facilitate for claims
