Request for Medical Report

How to apply for a Medical Report?

If you are making claims from your insurance company or if you require a medical report, Health Information Management Services (HIMS) provides medical report processing service. The release of medical information is subject to official approval.  

You may submit your insurance form or apply for a medical report at:

What is required during the request for medical information?

  • Patient’s written consent (application form). Download it here: Consent for release of medical information
  • Patient's NRIC / Identification documents 
  • Medical Report Fees (see table below) 
  • Payment can be made at our counter or Business Office  
  • Other supporting documents, e.g. marriage certificate, birth certificate and letters of administration, as proof of the representative's relationship to the patient. 
Request for medical report will only be processed upon receipt of your documents, consent form (Consent for Release of Medical Information) or insurance form, and appropriate payment.

Who can request for a Medical Report?

  • A patient who is a minor may apply for a medical report with the consent of his/her parent or legal guardian 

  • If the patient is deceased or mentally incompetent, consent is required from the appointed representative of the estate. If there is no representative appointed for the estate, a separate Letter of Undertaking has to be completed by all immediate next-of-kin of the patient. Copies of the death certificate and other supporting documents e.g. marriage certificate, birth certificate, and letters of administration, as proof of your relationship to the patient must be submitted together with the application.

  • Interested third parties, with the consent of the patient.

  • Please include this Letter of Undertaking under request for medical report for deceased / mentally incompetent patient.

Timeline for request completion

Please allow 4 - 6 weeks for processing of requests. 
Delays in prompt processing may be the result of: 
  • Patient’s upcoming clinic appointments or if the patient has been warded 
  • Reports requested from several clinical departments 
  • The doctor is away on leave

Why do I need to pay for medical report/completion of insurance form/ copies of lab results?

The regular consultation and hospitalization fees paid do not include cost for medical report. A fee has to be levied for this additional service of providing medical reports to cover the hospital's administrative costs and the doctor's professional inputs. This is a standard practice across all Public Healthcare Institutions.
