Request for Medical Report

If you are making claims from your insurance company or if you require a medical report, Health Information Management Services (HIMS) provides medical report processing service. The release of medical information is subject to official approval.


How to apply for a Medical Report?

By E-services (with SingPass)

The following report types are available for application through HealthHub.

    • Ordinary Medical Report / Ordinary Medical Report (PSY)
    • Completion of Insurance form / Completion of Insurance form (PSY)
    • Completion of Workman Compensation Form
    • Duplication of Investigation Results / Medical Certificate / Inpatient Discharge Summary
      (Note: Day Surgery Procedures do not have Discharge Summary)

  1. You may login with SingPass at HealthHub (under Medical Reports/Certificates Requests) website to submit request.
  2. Parents will need to add their child under Children's Health before submitting application for patient.
  3. You can upload insurance form or any other documents in PDF format in HealthHub.
  4. Payment to be made online via eNETS credit using Credit/Debit cards.

By Email

  1. You may email us your requests to medicalreports@skh.com.sg
  2. Please provide us these documents
    1. Completed Consent for Release of Medical Information Form Download it here: Consent Form - Release of Medical Information.pdf 
    2. Copy of patient's identification documents e.g. Copy of patient's NRIC (front and back)
    3. Any relevant supporting documents and forms.
  3. Your email will be responded to within three working days.
  4. We will register your request and advise you further payment instructions.

NOTE: Request for medical report will only be processed upon receipt of your documents, consent form (Consent for Release of Medical Information) or insurance form, and appropriate payment.

What is required during the request for medical information?

  • Patient’s written consent (application form). Download it here: Consent Form - Release of Medical Information 
  • Patient's NRIC / Identification documents 
  • Medical Report Fees (see table below) 
  • Payment can be made at our counter or Business Office  
  • Other supporting documents, e.g. marriage certificate, birth certificate and letters of administration, as proof of the representative's relationship to the patient. 

  • NOTE:
    Request for medical report will only be processed upon receipt of your documents, consent form (Consent for Release of Medical Information) or insurance form, and appropriate payment.

    Who can request for a Medical Report?

    • A patient who is a minor may apply for a medical report with the consent of his/her parent or legal guardian 

    • If the patient is deceased or mentally incompetent, consent is required from the appointed representative of the estate. If there is no representative appointed for the estate, a separate Letter of Undertaking has to be completed by all immediate next-of-kin of the patient. Copies of the death certificate and other supporting documents e.g. marriage certificate, birth certificate, and letters of administration, as proof of your relationship to the patient must be submitted together with the application.

    • Interested third parties, with the consent of the patient.

    • Please include this Letter of Undertaking under request for medical report for deceased / mentally incompetent patient.

    How soon can I obtain the requested medical information?

    As a general guide, please allow 4 - 6 weeks for processing of requests.
    However, we may experience a delay in processing some requests due to:

    • Patient's upcoming clinic appointments or if the patient has been warded
    • The doctor is away on leave 

    Why do I need to pay for medical report/completion of insurance form/copies of investigation results?


    The regular consultation and hospitalisation fees paid do not include the cost for medical report. A fee to be levied for this additional service of providing medical reports to cover the hospital's administrative costs and the doctor's professional inputs. This is a standard practice across all Public Healthcare Institutions.
